ABSTRACT

Los captadores de niebla son usados para interceptar agua contenida en la niebla y abastecer de agua a comunidades que habitan en lugares donde este recurso escasea. Se evaluó el uso de captadores de niebla para la captación de agua en un área ubicada en el páramo Pan de Azúcar, Duitama-Boyacá. Se instalaron 60 captadores de niebla, 24 de ellos, con dispositivos para medir los volúmenes de agua interceptados. Los volúmenes de agua captados, se midieron en periodos de 24 horas, por 26 días, durante un año y se usó el modelo geométrico para diferenciar el agua proveniente de la niebla. La precipitación registrada fue mayor a la reportada en la literatura. La precipitación mensual osciló entre 51 y 1198 mm y la temperatura media mensual entre los 6 y 8 °C. Los volúmenes de agua promedio colectados por los 24 captadores de niebla estuvieron entre los 0,02 Lm-2dia-1 hasta los 4,4Lm-2dia-1. Los aportes de agua provenientes de la niebla oscilaron entre los 0,02 y 1,77 mmdía-1. La dirección del viento no afectó la captación de agua y aún se presenta incertidumbre al separar el aporte real de agua proveniente de la niebla a partir de la lluvia orográfica, lo cual, sigue siendo un desafío en los ecosistemas de páramo, por lo que se debe ampliar la investigación, para mejorar los diseños y las eficiencias de los captadores de niebla.


Fog collectors are used to intercept water contained in fog and supply water to communities that live in places where this resource is scarce. We evaluated the use of mist collectors to collect water in an area located in the Pan de Azúcar paramo, Duitama-Boyacá. We installed 60 mist collectors, 24 of them with devices to measure the volumes of water intercepted. The volumes of water captured were measured in periods of 24 hours for 26 days during one year and we used the geometric model to differentiate the water from the fog. The recorded precipitation was higher than that reported in the literature. Monthly rainfall ranged between 51 and 1198mm and mean monthly temperature ranged between 6 and 8°C. The average volumes of water collected by the fog collectors were below 0.5Lm-2day-1 with a maximum of 4.4Lm-2day-1. The contributions of water from the mist ranged between 0.02 and 1.77 mmday-1. The direction of the wind did not affect the capture of water and there is still uncertainty when separating the real contribution of water from the fog from the orographic rain, which continues to be a challenge in the paramo ecosystems, for which it is necessary to expand research, to improve the designs and efficiencies of fog collectors.

ABSTRACT

SPeripheral sympathectomy is a procedure which has shown high rates of decreasing ischemic pain, recover functionality and wound healing, preventing the progression of the disease and further complications. We present a female patient with severe Raynaud´s phenomenon secondary to localized cutaneous systemic sclerosis complicated who presented digital ulcer treated with a sympathectomy of the radial and ulnar artery at the wrist level, undergoing post-operative follow-up.

Resumen Introducción: El objetivo del estudio fue validar el Cuestionario de Climas Motivacionales Empowering y Disempowering (Empowering and Disempowering Motivational Climate Questionnaire-Coach, EDMCQ-C) en jóvenes deportistas mexicanos a través del análisis de sus propiedades psicométricas, fiabilidad, validez convergente-discriminante, concurrente y factorial (de primero y segundo orden). Método: Participaron un total de 1243 jóvenes deportistas mexicanos de 12 a 17 años. Resultados: Los análisis mostraron adecuadas propiedades psicométricas, tanto de fiabilidad por dimensiones y factores, como de validez, convergente-discriminante, concurrente, factorial, de dos dimensiones (AFC de primer orden) y de cinco factores con sus respectivas dos dimensiones (AFC de segundo orden). Conclusión: Estos datos sugieren que la versión adaptada al contexto mexicano es un instrumento válido y fiable para evaluar la percepción de los climas motivacionales que generan los entrenadores en jóvenes deportistas mexicanos.


Abstract Abstract Introduction: The aim of the study was to validate the Empowering and Disempowering Motivational Climate Questionnaire-Coach (EDMCQ-C) in young Mexican athletes through the analysis of its psychometric properties, reliability, convergent-discriminant, concurrent and factorial (first and second order) validity. Method: A total of 1243 young Mexican athletes aged 12 to 17 years participated. Results: Analysis showed adequate psychometric properties, both reliability by dimensions and factors, as well as convergent-discriminant, concurrent, factorial, two-dimensional (first-order CFA) and five-factor validity with their respective two dimensions (second-order CFA). Conclusion: These data suggest that the version adapted to the Mexican context is a valid and reliable instrument to assess the perception of motivational climates generated by coaches in young Mexican athletes.

Introduction: Social stigmas in people living with Human Immunodeficiency Virus (HIV) have been part of their history and can still be identified in today's care environments. Objective: To analyze the social stigmas in relation to the care of people living with HIV by those who provide the anti Sars-CoV-2 vaccination service. Materials and methods: A descriptive quantitative research were developed that included a census sample of 126 people. The scale of social stigma in the care of people with HIV/AIDS (Acquired immunodeficiency syndrome) was applied. The information was analyzed through SPSS version 25. Results: Most of the participants reported not knowing people with HIV, but they have been treated as users (54%) and considered that they are discriminated (88.1%). Although a low level of stigma was evidenced (71.4%), these are present in 85.7%. Having had contact as a user with people with HIV was associated with the level of stigma (p=0.008). Conclusions: It is necessary to promote training in which the complexity of the phenomenon is addressed and aspects such as social stigmas are identified, incorporating the biopsychosocial paradigm together with a gender perspective in favor of the comprehensive provision of care.


Introducción: Los estigmas sociales en personas que viven con VIH han sido parte de su historia y aún se puede identificar en los entornos actuales de atención. Objetivo: Analizar los estigmas sociales en relación con la atención de las personas que viven con VIH por parte de quienes otorgan el servicio de vacunación anti Sars-CoV-2. Materiales y métodos: Se desarrolló una investigación cuantitativa descriptiva que incluyó una muestra censal de 126 personas. Se aplicó la Escala de estigma social en la atención de personas con VIH/SIDA. La información fue analizada a través del SPSS versión 25. Resultados: La mayoría de participantes refirió no conocer a personas con VIH, pero sí les han atendido como usuarios (54%), y consideró que son discriminadas (88,1%). Pese a que se evidenció un nivel bajo de estimas (71,4%), éstos están presentes en el 85,7%. El haber tenido contacto como usuario con personas con VIH se asoció con el nivel de estigma (p=0.008). Conclusiones: Se requiere fomentar capacitaciones en las que se aborde la complejidad del fenómeno y se identifiquen aspectos como los estigmas sociales, incorporando el paradigma biopsicosocial y con perspectiva de género en pro de la prestación integral de los cuidados.

BACKGROUND: Simple surrogate indexes (SSI) to assess beta-cell function, insulin sensitivity (IS) and insulin resistance (IR) are an easy and economic tool used in clinical practice to identify glucose metabolism disturbances. AIM: To evaluate the validity and reliability of SSI that estimate beta-cell function, IS and IR using as a reference the parameters obtained from the frequently sampled intravenous glucose tolerance test (FSIVGTT). MATERIAL AND METHODS: We included 62 subjects aged 20-45 years, with a normal body mass index and without diabetes or prediabetes. SSI were compared with the acute insulin response to glucose (AIRg), insulin sensitivity index (Si) and disposition index (DI) obtained from the FSIVGTT using the minimal model approach. Half of the participants (n = 31) were randomly selected for a second visit two weeks later to evaluate the reliability of all the variables. RESULTS: HOMA1-%B and HOMA2-%B had a significant correlation with AIRg (Spearman Rho (rs) = 0.33 and 0.37 respectively, p 0.50) with Si were fasting insulin, HOMA1-IR, HOMA2-IR, HOMA1-%S, HOMA2-%S, QUICKI, and the McAuley index. The parameters that showed good reliability with an intraclass correlation coefficient (ICC) > 0.75 were AIRg, HOMA1-%S, HOMA2-%S, and QUICKI. Conclusions: Our results suggest that most of the SSI are useful and reliable.


ANTECEDENTES: Los índices simples subrogados (ISS) que evalúan la función de célula beta, sensibilidad a la insulina (SI) y resistencia a la insulina (RI) son herramientas sencillas y económicas que se usan en la práctica clínica para identificar alteraciones del metabolismo de la glucosa. OBJETIVO: Evaluar la validez y confiabilidad de ISS para estimar la función de célula beta, SI y RI usando como referencia los parámetros de la prueba de tolerancia a la glucosa intravenosa con muestreo frecuente (FSIVGTT). MATERIAL Y MÉTODOS: Se incluyeron 62 sujetos de 20-45 años, con índice de masa corporal normal y sin diabetes mellitus o prediabetes. Los ISS se compararon con la respuesta aguda de la insulina a la glucosa (AIRg), índice de sensibilidad a la insulina (Si) e índice de disposición (DI) obtenidos de la FSIVGTT en base al modelo mínimo. La mitad de los participantes (n = 31) se seleccionaron aleatoriamente para acudir dos semanas después y evaluar la confiabilidad de todas las variables. RESULTADOS: HOMA1-%B y HOMA2-%B presentaron una correlación significativa con AIRg (Rho de Spearman (rs) = 0,33 and 0,37, respectivamente, p 0,50) con Si fueron insulina en ayuno, HOMA1-IR, HOMA2-IR, HOMA1-%S, HOMA2-%S, QUICKI y el índice de McAuley. Los parámetros que tuvieron buena confiabilidad (coeficiente de correlación intraclase > 0,75) fueron AIRg, HOMA1-%S, HOMA2-%S y QUICKI. Conclusiones: La mayoría de los ISS son instrumentos útiles y confiables.

ABSTRACT

RESUMEN INTRODUCCIÓN: La covid-19 afecta principalmente al aparato respiratorio, sin embargo, también se ha descrito afectación tanto directa como indirecta en el sistema nervioso central y periférico, lo cual ocasiona una gran variedad de manifestaciones neurológicas, siendo la encefalopatía una de las más frecuentemente observadas. OBJETIVO: Se busca mostrar la utilidad del video-electroencefalograma (vEEG) en el diagnóstico de encefalopatía en pacientes ingresados por covid-19, así como su valor para determinar el pronóstico de estos pacientes. MÉTODOS: Estudio observacional retrospectivo con 76 vEEG de 41 pacientes con covid-19 confirmada. Los estudios se han realizado entre los meses de marzo del 2020 y junio del 2021. Se estudió la gravedad de la enfermedad, así como sus características clínicas y neurológicas, el tratamiento farmacológico y los hallazgos electroencefalográficos según el grado de disfunción de la encefalopatía que desarrollaron estos pacientes. RESULTADOS: De los 41 pacientes, 12 (29 %) presentaron signos electroencefalográficos de disfunción cerebral leve, 15 (37 %) disfunción cerebral moderada y 14 (34 %) disfunción cerebral severa, los cuales se asociaron con una mayor mortalidad. CONCLUSIONES: En los 76 vEEG realizados a los 41 pacientes ingresados con encefalopatías asociadas con infección por covid-19, no se observó un patrón distinto a los descritos en encefalopatías de otras etiologías. El vEEG fue útil para confirmar la sospecha clínica de una disfunción cerebral en pacientes con encefalopatías asociadas con infección por covid-19 y para asignarle un grado de severidad, confirmando su beneficio como biomarcador diagnóstico y pronóstico.


ABSTRACT INTRODUCTION: COVID-19 mainly affects the respiratory system; however, both direct and indirect involvement of the central and peripheral nervous system has also been described, causing a wide variety of neurological manifestations, with encephalopathy being one of the most frequently observed neurological manifestations. OBJECTIVE: With this article we intend to show the usefulness of vEEG in the diagnosis of encephalopathy in patients referred for COVID-19 who develop this neurological complication, as well as its value in determining the prognosis of these patients. METHODS: Retrospective observational study with 76 video-electroencephalograms of 41 patients with confirmed COVID-19 infection. The studies were performed during the months of March 2020 through June 2021. Disease severity, clinical and neurological features, pharmacological treatment and electroencephalographic indings were studied according to the degree of encephalopathy dysfunction these patients developed. RESULTS: Of the 41 patients, 12 (29 %) presented electroencephalographic signs of mild cerebral dysfunction, 15 (37 %) moderate cerebral dysfunction, and 14 (34 %) severe cerebral dysfunction, which were associated with higher mortality. CONCLUSIONS: In the 76 vEEG performed in the 41 patients admitted with encephalopathies associated with COVID-19 infection, no pattern different from that described in encephalopathies of other etiologies was observed. The vEEG was useful to confirm the clinical suspicion of brain dysfunction in patients with encephalopathies associated with COVID-19 infection and to assign a degree of severity, confirming its benefit as a diagnostic and prognostic biomarker.

ABSTRACT Background: Insulin resistance is key in the pathogenesis of the metabolic syndrome and cardiovascular disease. Objective: We aimed to identify glucose and insulin patterns after a 5-h oral glucose tolerance test (OGTT) in individuals without diabetes and to explore cardiometabolic risk factors, beta-cell function, and insulin sensitivity in each pattern. Methods: We analyzed the 5-h OGTT in a tertiary healthcare center. We identified classes using latent class trajectory analysis and evaluated their association with cardiometabolic risk factors, beta-cell function, and insulin sensitivity surrogates by multinomial logistic regression analysis. Results: We included 1088 5-h OGTT performed between 2013 and 2020 and identified four classes. Class one was associated with normal insulin sensitivity and secretion. Class two showed hyperglycemia, dysinsulinism, and a high-risk cardiometabolic profile (obesity, hypertriglyceridemia, and low high-density lipoprotein [HDL] cholesterol). Class three included older individuals, a higher proportion of males, and a greater prevalence of hypertension, hyperglycemia, hyperinsulinemia, and postprandial hypoglycemia. Finally, class four showed hyperglycemia, dysinsulinism, and hyperinsulinemia; this class had the worst cardiometabolic profile (a high proportion of males, greater age, hypertension, obesity, hypertriglyceridemia, and low HDL cholesterol, p < 0.001 vs. other classes). Conclusions: The latent class analysis approach allows the identification of groups with an adverse cardiometabolic risk factor, and who might benefit from frequent follow-ups and timely multidisciplinary interventions.

Introducción: la pandemia por COVID-19 ha tenido un gran impacto y alto costo en la educación de los médicos en formación que no ha sido determinado a largo plazo. Objetivo: determinar el impacto del COVID-19 en médicos internos de pregrado y cómo repercutió en su formación académica. Material y métodos: estudio mixto en el que se revisaron las calificaciones del examen departamental bimestral del 2018 al 2020 de los médicos internos Facultad de Medicina de la Universidad Nacional Autónoma de México; se utilizó la técnica de grupos focales para indagar su experiencia y repercusiones durante la pandemia. Resultados: el promedio de calificaciones de los años 2018-2019 fue de 70.76 +/- 10.12. En el 2020 de 76.19 +/- 10.66 (p = 0.000). En la CDMX durante 2018-2019 se registró una media de 70.40 +/- 10.15 frente a 71.97 +/- 10.28 de las sedes foráneas (p > 0.05). En el 2020, las sedes de la capital tuvieron una media de 77.02 +/- 10.31 frente a 73.86 +/- 11.296 de las del interior (p = 0.000). Al comparar las calificaciones bimestrales, no hubo diferencias significativas en 2018 y 2019; sin embargo, en el 2020 hubo una elevación de 69.40 +/- 9.538 a 79.39 +/- 10.70. Conclusiones: se registraron mejores resultados en las calificaciones del 2020 en relación con las del 2018 y 2019, estas fueron más altas en las sedes capitalinas en relación con las foráneas. La percepción retrospectiva de los internos fue que dispusieron de más tiempo para el estudio de contenidos temáticos y menor presión en el trabajo asistencial; declararon que hubieran preferido quedarse en sus rotaciones, con el equipo de protección necesario para evitar contagiarse.


Background: The COVID-19 pandemic has had a great impact and hig+h cost on the education of physicians that has not been determined in the long term. Objective: To determine the impact of COVID-19 on undergraduate interns and how it affected their academic training. Material and methods: Mixed study. Qualifications of the bimonthly departmental exam from 2018 to 2020 of the FACMED-UNAM internal doctors were reviewed, the focus group technique was used to investigate their experience and repercussions during the pandemic. Results: The grade point average for 18-19 was 70.76+/10.12. In 2020 76.19+/-10.66 (p = 0.000). In the CDMX 2018-2019 an average 70,405+/-10,156 against 71.97+/10.28 of foreign headquarters (p > 0.05). In 2020, the capital's headquarters an average of 77.02+/-10.31 against 73.86+/-11.296 in the interior (p = 0.000). When comparing the bimonthly ratings, there were no significant differences in 2018 and 2019; However, in 2020 there is an increase from 69.40 +/-9.538 to 79.39 +/-10.709. Conclusions: The ratings for 2020 had better results in relation to 2018 and 2019, they were higher in capital cities in relation to foreign ones. The retrospective perception of the inmates was that they had more time to study thematic contents and less pressure in the care work; They stated that they would have preferred to stay in their rotations, with the necessary protective equipment to avoid getting infected.

ABSTRACT

RESUMEN El diseño de cebadores es fundamental para amplificar regiones de genes debido a que la especificidad que mantienen cebador-secuencia de interés puede causar el éxito o fracaso en la reacción de PCR. En relación a Potamotrygon magdalenae (especie de interés de acuerdo al PAN Tiburones-Colombia), existe poca información disponible de aspectos relacionados con la genética poblacional de esta Raya. El objetivo del presente trabajo consistió en diseñar cebadores bajo los criterios del Modelo de Pérdida de ADN (DNA-LM), que permitan evaluar el estado genético de las poblaciones de P. magdalenae. Alineamos secuencias de la superfamilia Dayastoidea, disponibles en el NCBI, de los genes mitocondriales Citocromo C Oxidasa 1 (MT-CO1) y Citocromo b (MT-CYB). Seguimos los parámetros Gap open penalty (5), Gap extension penalti (0,2) y Terminalgap penalties (0,1) y seleccionamos dos pares de cebadores de acuerdo con el DNA-LM. Estimamos el producto amplificado del gen MT-CO1 en 916 pb y del gen MT-CYB en 774 pb, en muestras de P. magdalenae procedentes de diferentes ciénagas del Magdalena medio. Discutimos los resultados desde la perspectiva de validar la especificidad de los cebadores diseñados, teniendo en cuenta la correspondencia e identidad de las secuencias de los genes considerados. Los cebadores aquí reportados pueden contribuir a ampliar el conocimiento de la genética poblacional, biogeografía y filogenética de la raya de agua dulce P. magdalenae.


ABSTRACT The primer design is critical for amplifying gene regions because the specificity that primer-sequence maintain can cause success or failure in the PCR reaction. Concerning Potamotrygon magdalenae (a species of interest according to the PAN Tiburones-Colombia), there is little information available about aspects related to population genetics of this river stingray. This study aimed to design primers under DNA Loss Model (DNA-LM) criteria, which will allow assessing the genetics status of populations of P. magdalenae. We aligned sequences from the superfamily Dayastoidea, available in the NCBI, of the mitochondrial genes Cytochrome C Oxidase 1 (MT-CO1) and Cytochromeb (MT-CYB). We followed the parameters Gap open penalty (5), Gap extension penalty (0.2) and Terminal gap penalties (0.1) and selected two pairs of primers according to the DNA-LM. We calculated the amplified product of the MT-CO1 gene in 916 pb and the MT-CYB gene in 774 pb in samples from different swamps of the middle Magdalena. We discussed the results from the perspective of validating the specificity of the primers designed, considering the correspondence and identity of gene sequences considered. The primers reported here will contribute to broadening the knowledge of population genetics, biogeography and phylogenetic of the river stingray P. magdalenae.

Abstract Objective: The objective of this study was to describe the case of a 19-year-old male presenting with bradycardia and hypotension after a honeybee sting making a review of the literature and pathophysiology of the cardiovascular and electrocardiogram (EKG) changes after a bee sting. Methods: The patient's airway was inspected and secured. Electrocardiogram with an idioventricular rhythm at 41' bpm. Oxygen was administered, an intravenous access was established, and the transcutaneous pacemaker leads were placed on the chest of the patient, then published guidelines management was induced. Results: The EKG showed idioventricular rhythm at a rate of 41 beats/min that resolved to a normal sinus rhythm after treatment. Conclusions: A full and prompt cardiovascular evaluation should be performed in all patients presenting to the emergency department after a bee sting, and published guidelines regarding the management of bradycardia and anaphylaxis should be followed to achieve successful outcomes.


Resumen Objetivo: Describir el caso de un paciente masculino de 19 años que presenta bradicardia e hipotensión después de una picadura de abeja haciendo una revisión de la literatura y fisiopatología de los cambios cardiovasculares y electrocardiográficos después de una picadura de abeja. Métodos: Se inspeccionó y aseguró la vía aérea del paciente. Un rastreo de ECG realizado al ingreso reveló ritmo idioventricular a una ritmo de 41 latidos por minuto. Se administró oxígeno, se estableció un acceso IV y se colocaron los cables del marcapasos transcutáneo en el tórax del paciente, luego se indujo el manejo de las guías publicadas. Resultados: El electrocardiograma mostró un ritmo idioventricular a una frecuencia de 41 latidos por minuto que se resolvió a un ritmo sinusal normal después del tratamiento. Conclusiones: Se debe realizar una evaluación cardiovascular completa y rápida en todos los pacientes que se presentan al departamento de emergencias después de una picadura de abeja, y se deben seguir las pautas publicadas sobre el manejo de la bradicardia y la anafilaxia para lograr resultados exitosos.

ABSTRACT Background: Bone marrow evaluation (BME) is crucial for establishing an accurate staging and prognosis in lymphoma patients. Objective: The objective of the study was to study the diagnostic performance of 18F-fluorodeoxyglucose positron emission tomography-computed tomography (FDG PET-CT) against bone marrow biopsy (BMB) for BME. Methods: Five hundred patient files of newly diagnosed lymphoma patients treated at an academic medical center were reviewed for BME at diagnosis by BMB and FDG PET-CT. Diagnostic performance of FDG PET-CT for detecting bone marrow infiltration (BMI) was assessed, as well as clinical predictors for positive BMB and positive FDG PET-CT. Results: BMB was positive in 16.3% of all patients, and 28.7% had a positive FDG PET-CT for BMI. Overall, the sensitivity of FDG PET-CT was 74.1% and specificity 80.1%. As for predictors for BMB and FDG PET-CT positivity, B symptoms and thrombocytopenia were independent factors for BMI. Seventy-four patients had discordant results between BMB and FDG PET-CT, non-Hodgkin lymphoma (NHL) having the most significant discordance. This discrepancy did not affect treatment. Conclusions: FDG PET-CT shows excellent performance for the detection of BMI in Hodgkin lymphoma. For diffuse large B-cell lymphoma, we recommend performing BMB and FDG PET-CT as complementary tests. In all other NHL, a unilateral BMB is mandatory at diagnosis.

ABSTRACT

The Coronavirus pandemic (COVID-19) triggered a global health emergency, and as a response, institutions and medical services have implemented different measures regarding the management of these patients in order to maintain medical care, and reduce the spread of the virus in patients and medical staff. The following review aims to present the current recommendations made by international, national and local societies regarding medical action from Traumatology and Orthopaedics. A literature review was done on Medline / Pubmed platforms, The Journal Bone and Joint Surgery (JBJS), Journal of the American Academy of Orthopedic Surgeons (JAAOS), Sociedad Chilena de Ortopedia y Traumatología (SCHOT). The literature was in Spanish and English, comparing it with reality on a local level. The current COVID-19 pandemic led to the complete reform of the trauma care units, starting with the development of subspecialty teams that rotate periodically, which are in charge of the emergency area, treating patients with all personal protection elements (PPE) and categorizing those patients who require emergency trauma surgery, from those that can be managed in a delayed manner. Regarding patients who are hospitalized, a COVID-19 PCR (Polymerase Chain Reaction) test is taken as screening, except for emergency surgeries where all intraoperative protection measures are taken. In addition, for greater safety, all elective surgery was suspended, which reduces the flow of patients in the ward and in the inpatient unit. Outpatient care was optimized in order to protect patients in their periodic check-ups.

Resumen Objetivo: medir el impacto de la terapia de resincronización cardiaca en términos de variables ecocardiográficas en pacientes de países latinoamericanos. Método: se realizó un estudio prospectivo, multicéntrico, intervencionista, en el que los pacientes elegibles fueron llevados, por primera vez, a implante de un dispositivo de resincronización cardiaca. El objetivo primario fue valorar los cambios del tamaño y la función del ventrículo izquierdo por medio de un ecocardiograma previo al implante del dispositivo y en el sexto mes. Los objetivos secundarios evaluados fueron hospitalizaciones, cambios en la clase funcional, mortalidad, calidad de vida y un score compuesto clínico basado en estos factores de evaluación global del paciente. Resultados: para cumplir el objetivo primario se analizaron datos de 75 sujetos. La edad promedio fue de 63,7 años; 21.3% fueron mujeres y 30.7% tuvieron cardiopatía isquémica. Al sexto mes de seguimiento las mediciones de volumen de fin de diástole y sístole del ventrículo izquierdo disminuyeron en promedio 37.6 ml y 37.8 ml, respectivamente. La fracción de eyección del ventrículo izquierdo en promedio se incrementó un 11%. El puntaje compuesto clínico mostró mejoría en el 86.4% de los pacientes en el sexto mes postimplante del resincronizador. Conclusiones: se observó remodelado inverso del ventrículo izquierdo y mejoría en el estado clínico de los pacientes con insuficiencia cardiaca y disfunción sistólica del ventrículo izquierdo que recibieron terapia de resincronización cardiaca en el ámbito de la práctica clínica de rutina.


Abstract Objective: To measure the impact of cardiac resynchronisation therapy in terms of cardiac ultrasound variables in patients from Latin-American countries. Method: A prospective, multicentre, interventionist study was conducted, in which the eligible patients were those that had a cardiac resynchronisation device implanted for the first time. The primary objective was to assess the changes in size and left ventricular function by means of a cardiac ultrasound carried out prior to implanting the device and in the sixth month. The secondary objectives evaluated were hospital admissions, change in functional class, mortality, quality of life, and an overall assessment of the patient using a combined clinical score based on these factors. Results: A total of 75 subjects were analysed in order to complete the primary objective. The mean age was 63.7 years; 21.3% were female, and 30.7% had ischaemic heart disease. At the sixth month, the left ventricular end-diastolic and systolic volume decreased by a mean of 37.6 ml and 37.8 ml, respectively. The left ventricular ejection fraction increased by a mean of 11%. The combined clinical score showed an improvement in 86.4% of the patients in the sixth month after the implantation of the synchronisation device. Conclusions: A reverse remodelling of the left ventricle was observed, as well as an improvement in the clinical stage of patients with heart failure and left ventricular systolic dysfunction that received cardiac resynchronisation treatment in the setting of routine clinical practice.

RESUMEN La gastroenteritis eosinofílica (EG) es una entidad poco frecuente. Presentamos un varón de 55 años sin antecedentes de atopía ni alergia, que presentó dolor abdominal y vómitos. La tomografía computarizada abdominal y la ecoendoscopia demostraron engrosamiento de la pared gástrica con engrosamiento de la pared del duodeno superior, sin masa definida ni colección de líquido. La endoscopia gastrointestinal confirmó engrosamiento de pliegues prepilóricos y duodeno superior con estenosis luminal. Se realizó gastrectomía parcial por ausencia de diagnóstico definitivo y sospecha de posible malignidad y diagnóstico de obstrucción de la salida gástrica. La histopatología fue compatible con EG, tratada con un tratamiento con corticoides de 8 semanas de duración, mostró mejoría clínica, aumento de peso y normalización del recuento de eosinófilos en sangre periférica.


ABSTRACT Eosinophilic gastroenteritis (EG) is a rare entity. We report a 55-year-old man had no previous atopy or allergic history, who presented abdominal pain and vomiting. Abdominal computed tomography and endoscopy ultrasonography demostrated gastric wall thickening with wall thickening of the superior duodenum, without defined mass or fluid collection. Gastrointestinal endoscopy confirmed thickening of prepyloric folds and superior duodenum with luminal stenosis. Parcial gastrectomy was performed due to absence of definitive diagnosis and suspicion of possible malignancy and diagnosis of gastric outlet obstruction. Histopathology was compatible with EG, treated with an 8-week long corticosteroid therapy, showed clinical improvement, weight gain and normalization of eosinophil count on peripheral blood.

ABSTRACT

Flexor tendon tenosynovitis is an entrapment of the flexor tendons at its entrance to the pulley system. Because there is a high incidence of this pathology, it should be well known by physicians, rheumathologists and orthopaedic surgeons. On this paper we present a literature review, analyzing the anatomic facts, biomechanics, diagnosis, classification, therapeutic options and we propose some general recommendations for physicians.
